"Model in upper right on front --> MPCR6793PS

Has a PINE sticker on back. <-- This was the key.

Has FM-3711-11 and RockwellR 6793 PCI Modem on Manufacter sticker on back also.

Conexant chipset. RS56/SP-PCI"
